May need to extend localities beyond the Arfon Locality as this may be only provide a very small cohort of patients. Gaining full clinical engagement of all the ward staff and other localities.
The patient will be discharged a couple days early with the MyMedic11 monitoring. This will reduce length of stay and reduce re-admission rates. Additionally it will assist the patient towards understanding their condition and provide reassurance.
